The key information I will need at the appointment for each employee is the following information: Employee full name Employee's date of birth Employee's last four of social security numbers Male or female gender Home zip code Whether the plan will be for solely the employee only or the employee plus a spouse or employee plus children or employee plus spouse and children If anyone other than the employee wants to be included in the employee group plan then please provide each individual person's full name and date of birth and gender. If the employee does not want to participate in the group coverage due to already having health insurance coverage through Medicare or veteran health benefits, a waiver must be signed and the reason provided Please inform us if the employee wants dental and/or vision benefits
